Consent of the subject of personal data to the collection and processing of his personal data

Freely, consciously, of my own free will, I consent to processing (any operation or a set of operations performed independently by the holder (owner) of personal data or on his behalf, automatic means or without them, for the purpose of collecting, recording, storing, updating, grouping, blocking, erasure and destruction of personal data), as well as for transfer to third parties in accordance with the Law of the Kyrgyz Republic "On personal information" the following personal data:

• Last name, first name and patronymic;
• Personal identification number (PIN);
• Date of Birth;
• Sex;
• Information about citizenship;
• Taxpayer Identification Number (TIN);
• Name of the legal entity;
• E-mail address;
• Cell phone number;

I provide the above personal data for processing for the purposes of providing me with government services in the field of production licensing (production) and sale of veterinary medicines.

I am aware that consent to the processing of personal data is valid with from the moment of registration in the licensing system for the entire period providing me with government services in the field of production licensing (production) and sale of veterinary medicinal products and storage data on the service provided in accordance with the legislation of the Kyrgyz Republic Republic
